Qualifications

• 2+ years of experience in health care, care management, or related field.
• RN license active and unrestricted in the state of practice.
• Valid driver’s license with reliable transportation for travel requirements.
• Understanding of EMR and HIPAA regulations.
• Ability to work proactively with attention to detail.
• Strong written and verbal communication skills.
• Willingness to travel 25–40% locally as needed.

Responsibilities

• Completes comprehensive member assessments per regulated timelines and determines care management eligibility.
• Develops and implements a care coordination plan with member, caregiver, physician, and team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions, and adjusts as needed.
• Maintains ongoing member caseload with regular outreach and follow-up.
• Facilitates interdisciplinary care team meetings and coordination.
• Uses motivational interviewing to support member behavior change.
• Assists with barriers to care and provides resources to members.
