Qualifications

Responsibilities

• Complete comprehensive member assessments within regulatory timelines.
• Develop and implement care coordination plans with members and care network.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on outcomes and interventions.
• Maintain ongoing member caseload and regular outreach.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate and motivate members using motivational interviewing and clinical guidelines.
• Identify barriers to care and coordinate resources for members.
