Privia Health is seeking a dedicated RN Care Manager to partner with Maryland primary care providers in delivering Comprehensive Collaborative Care to patients with multiple chronic conditions. The role emphasizes patient-centered coordination across care teams and community resources.

The ideal candidate holds an RN or LPN license with 2+ years in population health or care coordination, strong EHR skills, and willingness to travel within the DMV.

Qualifications

• 2+ years of population health or care coordination experience.
• RN or LPN license in MD or ability to obtain.
• Strong computer and EHR skills.

Responsabilités

• Collaborate with care teams to meet Maryland Primary Care Program metrics.
• Coordinate care plans and perform outreach for disease management.
• Provide Transitional Care Management after hospital or ED discharge.
• Educate patients and assist with accessing necessary care outside the Primary Care Office.
• Identify and coordinate referrals to community resources and home care.
• Travel to care centers to meet patients and providers as needed.
• Attend regular care team meetings and participate in performance reviews.
• Operate independently within defined patient populations to achieve goals.
