Maryland Physicians Care Inc. is seeking a Registered Nurse to lead the planning, development, implementation, and timely oversight of care plans for high risk and special needs members. The role aims to improve coordination, continuity, accessibility, and appropriate utilization of health care services and community resources.

Maryland Care Management, Inc. (MCMI) supports MPC's statewide provider network, serving over 230,000 members.

Qualifications

• RN with current state license.
• 2+ years of clinical nursing experience in acute care or community setting.
• 2 years of care management experience.
• Knowledge of Managed Care Programs preferred.
• BSN preferred.
• CCM preferred.

Responsibilities

• Assists in developing, maintaining, and monitoring processes to promote timely identification and assessment of high risk and special needs members.
• Serves as an interdisciplinary team member in management of identified members, collaborating with physicians to determine medical needs.
• Conducts interviews of members to identify risk factors and need for care management.
• Educates members' medical providers regarding members' special needs to ensure appropriate treatment planning.
• Coordinates with community support and social service systems for members.
