LifeBridge Health in Baltimore seeks an RN Care Coordinator to enhance patient health outcomes by coordinating care across multiple settings. This role involves education for patients and caregivers on health management, requiring 3-5 years of acute care nursing experience and a nursing degree.
The RN Care Coordinator will play a key role in reducing care costs while improving quality for high-risk patients, within a dynamic healthcare system committed to community-centered care.
The RN Care Coordinator will work closely with primary care physicians and various specialty physicians and services to maximize the health status of patients. This position requires frequent contact with the patient and their caregivers for education of health promotion and self‑care skills. The RN Care Coordinator will facilitate the medical care of the patient between all care settings including, but not limited to, inpatient, outpatient, ambulatory, and primary care settings. This role will be instrumental in decreasing the cost of care while increasing the quality of care for the high‑risk patient.
3-5 years demonstrated proficiency in acute care nursing, knowledge and skills.
Direct experience in acute care, ambulatory care, or payer‑based case management strongly desired. Working knowledge of post‑acute levels of care necessary.
Education Required: Nursing Degree
Required: Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license.
AHA Basic Life Support
Preferred: Case Manager Certification
