Job summary

Qualifications

• 3-5 years of direct practice experience in inpatient, partial, and/or outpatient settings preferred.
• Working knowledge of value based care models such as ACOs, PCMH, and MACRA/MIPS.

Responsibilities

• Manage care transitions from ER, hospital, and nursing home.
• Coordinate individual care planning for high risk patients.
• Implement Clinical Quality Improvement (CQI) activities.
• Provide patient education plans and validate understanding.

Education

B.S. Degree in Nursing

Current valid and unrestricted medical license in Maryland (RN, Ph.D, LMSW/LCSW, MHC, LLP, LPC)
