Job summary

Privia Health is seeking a Nurse Care Manager for our onsite New London office in Forest, VA to deliver expert care coordination for high-risk patients. You will work with population health teams, PCPs, and care centers to implement and monitor comprehensive care plans.

The role emphasizes wellness visits, chronic disease management, and close collaboration with patients and providers to reduce high-cost utilization while improving outcomes.

Qualifications

• BS in Nursing (or equivalent) required.
• Active unrestricted nursing license in Virginia.
• Strong computer and EHR skills and experience.
• HIPAA compliance knowledge.

Responsibilities

• Manage a diverse caseload to optimize health outcomes and patient satisfaction.
• Coordinate care across PCPs and care teams to reduce high-cost utilization.
• Conduct wellness visits for Medicare patients and educate on care plans.
• Use Athena and Privia apps to identify opportunities and close quality gaps.
• Adhere to RN supervision for assessments and care planning.
