Job summary

Molina Healthcare, Inc. is hiring a Care Manager RN to coordinate long-term services and supports for high-need members. You will work with a multidisciplinary team to ensure quality, cost-effective care across the continuum.

Responsibilities include assessments, waiver management, care plan development, and monitoring outcomes, with travel 25-40% based on state contracts. Must hold an active RN license and have strong communication skills.

Qualifications

• 2+ years healthcare experience including 1+ year in care management or LTSS
• RN license active and unrestricted in state of practice
• Bachelor's degree in health care field may be required in some states
• Valid driver's license and transportation
• Excellent communication and organizational skills
• Experience with populations with disabilities or chronic conditions can be beneficial

Responsibilities

• Complete comprehensive member assessments within required timelines including in-person visits
• Facilitate waiver enrollment and disenrollment processes
• Develop and implement care plans with members, caregivers and professionals
• Monitor care plans and adjust interventions to meet goals
• Coordinate with ICT meetings for approvals or denials
• Educate and motivate members using motivational interviewing
• Identify barriers and coordinate resources to address psycho/social and medical needs
• Travel locally 25-40% as required by state contracts
