Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, families, physicians and staff.
• Monitor care plans and evaluate effectiveness; adjust as needed.
• Promote integration of LTSS with behavioral health and community resources.
• Facilitate ICT meetings for service decisions and documentation.
• Adhere to regulatory requirements and ensure quality and cost-effective care.
