Molina Healthcare, Inc. is seeking an RN-based care manager to support LTSS-related activities and care coordination. You will collaborate with multidisciplinary teams to ensure integrated member care and progress toward outcomes.

The role involves comprehensive assessments, waiver processes, care plan development, and ongoing monitoring. Travel 25–40% may be required depending on state contract requirements, with a focus on delivering cost-effective, high-quality care.

Qualifications

• 2+ years in health care, including care management or managed care experience.
• RN license active and unrestricted.
• Bachelor's degree may be required in some states.
• Valid driver's license with reliable transportation for travel requirements.
• Ability to work independently and manage multiple projects.

Responsibilities

• Perform comprehensive member assessments and home visits as needed.
• Coordinate waiver enrollment/disenrollment and care plans with members and care networks.
• Coordinate and monitor care plans, evaluating outcomes and adjusting as needed.
• Support integration of behavioral health with LTSS and community resources.
• Assess medical necessity and authorize waiver services.
• Participate in ICT meetings for service approvals/denials.
• Travel 25–40% as required by state/contract.
