Job summary

Molina Healthcare, Inc. is seeking an RN Care Manager to support care coordination for members requiring long-term services and supports. You will work with an interdisciplinary team to develop, implement and monitor care plans, ensuring progress toward outcomes and optimal, cost-effective care.

You will complete in-home assessments, coordinate waiver services, and navigate eligibility and benefits. Expect travel 25–40% as required by state contracts; strong communication is essential.

Qualifications

• At least 2 years of health care experience, including 1 year in care management or LTSS.
• RN license active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Ability to work independently with minimal supervision and maintain accuracy.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care network.
• Monitor care plans and evaluate effectiveness, documenting interventions and outcomes.
• Promote integration of LTSS, behavioral health and home/community resources for continuity of care.
• Assess medical necessity and authorize waiver services; evaluate benefits and funding sources.
• Lead ICT meetings for approval/denial of services and interdisciplinary collaboration.
• Educate and motivate members using motivational interviewing and Molina guidelines.
• Identify barriers to care and assist with psycho/social, financial and medical obstacles.
• May provide consultation to peers; complex cases may involve medication reconciliation.
• Travel 25-40% depending on state/contractual requirements.
