Molina Healthcare, Inc. is seeking a Care Manager RN to support LTSS coordination and care management. You will work with a multidisciplinary team to ensure high-need members progress toward outcomes and receive cost-effective care across settings.

Responsibilities include comprehensive assessments, waiver enrollment, care-plan development, and ongoing monitoring. Travel up to 40% may be required based on state contracts.

Qualifikationen

• RN license active and unrestricted in state of practice.
• At least 2 years in healthcare, incl. 1 year in care management or LTSS.
• Valid driver's license and reliable transportation for travel.

Aufgaben

• Perform comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and develop care plans.
• Monitor care plan effectiveness and adjust as needed.
• Promote integrated LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
• Lead ICT meetings for service approvals and collaborate with teams.
• Travel 25–40% as required by state/contractual rules.
