Job summary

Molina Healthcare, Inc. is seeking a Care Manager RN to support care management and LTSS coordination across the continuum. You will collaborate with an interdisciplinary team to drive outcomes and cost-effective member care.

The position involves member assessments, care plan development, monitoring progress, and promoting service integration. Local travel may be required to meet state/contractual requirements.

Qualifications

• 2+ years health care experience including at least 1 year in care management or managed care.
• RN license active and unrestricted in state of practice.
• In some states, a bachelor’s degree in a health care related field may be required.
• Valid driver’s license and reliable transportation for travel.
• Ability to work independently and with diverse populations.
• Experience with LTSS and with individuals with disabilities or chronic conditions.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and health professionals.
• Monitor care plan effectiveness and document interventions and outcomes.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize appropriate waiver services.
• Coordinate interdisciplinary care team meetings for service decisions.
• Provide motivational interviewing and support to encourage change.
