Job summary

Molina Healthcare, Inc. is seeking a Care Manager RN to support care management and LTSS coordination, collaborating with multidisciplinary teams to ensure high-need members receive integrated, quality and cost-effective care.

You will perform comprehensive member assessments, develop and monitor care plans, facilitate waiver processes, and coordinate services across settings, including travel as required. This role requires strong communication, independent work, and the ability to navigate

Qualifications

• Minimum 2 years in health care with at least 1 year in care management or LTSS.
• Active RN license in state of practice.
• Bachelor's degree may be required depending on state/contract.
• Valid driver's license with reliable transportation for travel.
• Ability to work independently with minimal supervision.
• Strong communication and interpersonal skills.
• Proficient with Microsoft Office applications.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits as required.
• Develops and implements care plans and waiver service plans with members and care network.
• Monitors care plan progress, documents interventions, and suggests changes.
• Promotes integration of services across LTSS, behavioral health, and community resources.
• Assesses medical necessity; authorizes appropriate waiver services.
• Facilitates ICT meetings for service approvals/denials and collaboration.
• Travel 25-40% as required by state/contractual requirements.
