Job summary

Qualifications

• RN license is required and active in the state of practice.
• 2+ years in health care with care management or LTSS experience.
• Experience with disability and chronic condition populations is preferred.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, and professionals.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of LTSS, behavioral health, and community resources.
