Molina Healthcare, Inc. is seeking a Registered Nurse care manager to support care management and LTSS activities. The role partners with a multidisciplinary team to coordinate integrated member care across the continuum and ensure progress toward outcomes.

The position involves waivers, care plan development with members and caregivers, and ongoing monitoring. Travel to members' locations may be required and the role emphasizes proactive, compliant, and compassionate support.

Formación

• At least 2 years of experience in health care with care management exposure.
• Registered Nurse (RN) with active license in the practicing state.
• In some states, a bachelor's degree in a health care related field may be required.
• Valid and unrestricted driver's license and reliable transportation for job-related travel.
• Ability to work independently and manage multiple priorities in dynamic settings.

Responsabilidades

• Complete comprehensive member assessments within required timelines, including home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, families, and care team.
• Monitor care plan effectiveness and document progress toward goals.
• Promote integration of LTSS, behavioral health, and community resources.
