Job summary

Molina Healthcare seeks a Care Manager RN to support LTSS member care across a multidisciplinary team. You will conduct assessments, coordinate waiver services, and develop individualized care plans to improve outcomes while managing costs.

Ideal candidates have RN licensure, 2+ years in healthcare with care management experience, and strong communication skills to engage members, families, and providers in diverse settings.

Qualifications

• At least 2 years of experience in health care, including care management, managed care, or LTSS exposure.
• Registered Nurse (RN) license active and unrestricted in state of practice.
• Bachelor's degree in a healthcare-related field may be required depending on state/contractual requirements.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, families, and providers to address needs and goals.
• Monitor care plans for effectiveness and adjust as needed.
• Promote integration of services across LTSS, behavioral health, and community resources.
