Job summary

Molina Healthcare in Long Beach, CA is seeking a Care Manager RN to provide care coordination across the continuum. You will assess members, develop and monitor care plans, and collaborate with an interdisciplinary team to improve outcomes and reduce costs.

Responsibilities include telephonic and in-person visits, travel as needed (25-40%), and ensuring HIPAA compliance while coordinating services with behavioral health and LTSS resources.

Qualifications

• At least 2 years experience in health care, preferably in care management.
• RN license must be active and unrestricted.
• Valid driver's license with reliable transportation for travel.
• Understanding of EMR and HIPAA requirements.
• Knowledge of community resources and referral networks.
• Able to work independently with minimal supervision.
• Strong communication, time-management and multitasking skills.

Responsibilities

• Completes comprehensive member assessments per timelines and determines eligibility for care management.
• Develops care coordination plans with member, caregivers, physicians and other health professionals.
• Conducts telephonic, home visits or in-person assessments as required.
• Monitors and documents progress toward goals and adjusts plans as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to support change during contacts.
• Assesses barriers to care and provides coordination and assistance.
• May provide consultation and resources to peers as needed.
• Travel 25–40% may be required per state/contract.
