Qualifications

• Driver's license with reliable transportation for local travel.
• Strong understanding of HIPAA and EMR systems.
• Ability to work independently and manage multiple cases.

Responsibilities

• Perform comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with the care team.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor and adjust care plans based on outcomes and barriers.
• Manage a caseload and coordinate services across settings.
• Collaborate with ICT and community resources to ensure continuity.
• Educate and motivate members using motivational interviewing.
• Assist peers with consultation and resource sharing as needed.
