Job summary

Mercy is seeking a Care Manager in Fort Smith, AR to coordinate discharge planning and care transitions for patients. You will work with physicians, nurses, and interdisciplinary teams to ensure safe, timely patient journeys from admission through discharge.

Qualifications

• BSN degree required.
• Current RN license in Arkansas.
• 2–3 years of acute care hospital experience preferred.
• Care Management or Utilization Management experience preferred.
• BLS certification required at hire or within 90 days.
• Case Management certification preferred.
• Strong communication, critical thinking, and collaboration across disciplines.
