Job summary

VillageMD is seeking an RN Care Manager to collaborate with providers and engage high-risk patients to develop patient-centered care plans focused on chronic disease management and outcomes. You will work with Care Coordinators, Nurse Navigators and Social Workers to integrate care across services and settings.

Spend 2-3 days per week embedded in a physician office, partnering with care teams to set goals, coordinate interventions and support continuity of care.

Qualifications

• 3+ years of direct, clinical nursing experience.
• Registered Nurse with licensure in the state of practice.
• Care management experience or transitional care management is a plus.
• Proficiency with electronic health records and data reporting.

Responsibilities

• Collaborate with providers and care team to develop patient-centered care plans.
• Engage high-risk patients to improve health and reduce unnecessary utilization.
• Coordinate referrals and education to support continuity of care.
