Job summary

VillageMD seeks an RN Care Manager to collaborate with providers and engage high-risk patients in developing patient-centered care plans focused on chronic condition management and measurable outcomes. You will work with Care Management teams, including Nurse Navigators and Social Workers, to connect patient care across settings.

You will spend 2–3 days per week embedded in a physician office, engaging high-risk patients, and coordinating with the care team to improve overall health and reduce

Qualifications

• 3+ years of direct clinical nursing experience.
• Registered Nurse with licensure in the state of practice.
• Care management or transitional care management experience valued.

Responsibilities

• Embedded in a physician office 2–3 days per week, collaborating with providers.
• Engage high-risk patients to develop goals and action steps for wellness.
• Coordinate health education, referrals, and continuity of care with the care team.
