Qualifications

• Valid NYS RN license required.
• Minimum 3 years of experience in Managed Care, Home Care, or Community settings.
• Associate's degree or higher required.
• Bilingual proficiency in Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole.

Responsibilities

• Develop and manage care plans and coordinate services.
• Conduct health assessments and monitor progress.
• Oversee transitions and discharge planning.
• Identify risk factors and collaborate with the Interdisciplinary Care team.
