Job summary

BayCare Hospital in the Tampa Bay area is seeking a Hospital Care Coordinator-RN Case Management at St Josephs Hospital South in Riverview, FL. The role provides linkage, monitoring and advocacy to maximize functioning and coordinates discharge planning.

Requires RN license and 2 years of acute care experience; schedule Monday–Friday 8:00 AM - 4:30 PM with occasional weekends. Benefits include health, dental, vision, paid time off, tuition reimbursement, 401k match and annual performance bonuses.

Qualifications

• RN license required.
• ACM or CCM certifications preferred.
• Associates of Nursing or Bachelor of Nursing education.

Responsibilities

• Manages assigned caseload based on individual needs and preferences.
