Job summary
Visiting Angels Hoboken is seeking an experienced RN Care Manager to design person-centered home care plans, mentor caregivers, and advocate for high-quality, safe, and comfortable home-based care. You’ll lead in-home assessments, collaborate with families and clinicians, and drive compliant, client-centered outcomes.

Qualifications
- RN license valid in New Jersey required.
- Bachelor’s in Nursing or related human-services field; or 2+ years geriatric/home-care experience.
- Strong clinical assessment and problem-solving skills.
- CPR/First Aid certification and valid driver’s license.
- Warm, professional communication with families and staff.
Responsibilities
- Assess and plan: conduct in-home RN assessments and craft individualized care plans.
- Coach and supervise: orient caregivers to care plans and perform field competency checks.
- Monitor and document: track vitals, condition changes, and interventions in HIPAA‑secure EMR.
- Collaborate and advocate: coordinate with physicians, therapists, and schedulers.
- Quality and compliance: maintain audit-ready files and ensure regulatory adherence.
