Job summary

Promise Care NJ is seeking a compassionate Home Health Visiting Nurse to provide high-quality, in-home nursing care to patients. The RN will conduct assessments, administer medications, monitor progress, educate families, and coordinate with physicians and therapists, traveling regularly to patients’ homes.

The role requires a valid NJ RN license, minimum 1 year of home health experience, strong clinical skills, and excellent communication. BLS is required, ACLS preferred.

Qualifications

• Valid Registered Nurse (RN) license in the state of practice.
• Minimum of 1 year of nursing experience, preferably in home health or a related field.
• Strong clinical skills and the ability to make independent judgments.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced and ever-changing environment.
• BLS certification required, ACLS certification preferred.

Responsibilities

• Conduct thorough assessments of patients' health status and create individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patients' progress and update care plans as needed.
• Educate patients and their families on health management and self-care techniques.
• Coordinate care with other healthcare providers, such as physicians, pharmacists, and physical therapists.
• Provide emotional support to patients and their families during challenging times.
• Document all care provided and communicate effectively with the healthcare team.
