AdventHealth in the United States is seeking an experienced RN for home health. You will assess home environments, coordinate care, and deliver skilled nursing interventions to a diverse patient caseload.

Qualifications

• RN with an Associate's degree required.
• BSN preferred.
• 1+ year of clinical nursing experience in home health or Medicare-certified settings.
• Licensure: Registered Nurse (RN) and Driver's License.
• Basic Life Support (BLS) certification required.

Responsibilities

• Evaluate home environment for safety, infection control, and resource needs.
• Review patient history, diagnostics, and labs; report abnormalities to physician and plan care.
• Provide skilled nursing care and prescribed treatments in home settings.
• Use health coaching and motivational interviewing to engage patients and families.
• Maintain and update clinical records and ensure care plan updates are documented.
• Develop patient-specific care plans with patient, family, and physician; set measurable goals.
• Optimize daily schedules for productivity and best-practice visit utilization.
