Overview

The RN Care Manager II - IPCM collaborates with an interdisciplinary team to assess, plan, advocate for, and coordinate patient care from admission to discharge, ensuring safe post-hospitalization transitions. This involves evaluating patient health needs, creating individualized care plans, and coordinating services to improve health outcomes. Effective communication with patients, families, and healthcare providers is essential. The role also involves advocating for patient needs, promoting health education, and facilitating seamless care transitions across settings. Clinical expertise, critical thinking, and compassion are vital to support patient well-being and healthcare improvements. All duties align with Mercy’s mission, values, and service standards.

Qualifications
  • Education: Bachelor’s Degree of Science in Nursing, required
  • Licensure: Current RN License in the state of employment, required
  • Experience: 1 year of acute care hospital setting, required
  • Required Certifications: BLS (Basic Life Support) at hire date or within 90 days of hire
  • Preferred Certification: Certification in Case Management
