Job description

Title: LTSS Service Care Manager

Location: Region - Must reside within Ft. Myers (Lehigh Acres & Ft Myers City)

Will be in the field 80-90% of the time.

Duration: 06 Months (Potential to extend)

Shift: 8:00 AM - 5:00 PM

Position Purpose:

Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.

Education/Experience:

Requires a Bachelor's degree and 2 - 4 years of related experience or equivalent experience reflecting the duties, scope, and skills of this position.

License/Certification:

For Iowa: Bachelor's degree with 30 semester hours in a human services field and at least two years of experience, or a current RN license with at least four years of experience.

For North Carolina: Two years of LTSS and/or HCBS coordination, care delivery monitoring, and care management experience; RN or LCSW required; LCSW-A preferred.

Responsibilities:
  1. Evaluate member needs and available resources to recommend the best plan.
  2. Develop ongoing long-term care plans and identify providers and community resources.
  3. Coordinate between members, families, caregivers, and care providers to ensure service accessibility.
  4. Provide resource support based on assessments and plans.
  5. Monitor care and member outcomes, and adjust plans accordingly.
  6. Interact with healthcare providers and partners to meet member needs.
  7. Maintain accurate documentation to ensure compliance.
  8. Perform home or site visits as needed.
  9. Educate members and families on procedures, services, and benefits.
  10. Provide feedback for quality improvement.
Additional Details:
  • Schedule: 8-5 Mon-Fri; includes travel (~80%); remote role; valid driver's license required.
  • Manage a caseload of healthcare members with long-term care needs.
  • Conduct assessments, monitor service delivery, and coordinate referrals.
  • Assist with discharge planning and ensure seamless service delivery.
Education/Certification:

Required: Bachelor's degree in healthcare-related field and 2-4 years of experience. Field experience must be extensive.

Required: Valid driver's license.

Preferred: n/a

Must Haves:
  • 2+ years of Care Management experience in the field.
  • Experience managing high caseloads of 50-70 members aged 65+.
  • Long Term Care Medicaid experience.
  • Medicaid/Medicare experience.
  • Comfortable managing high caseloads and working in a fast-paced environment.
  • Proficiency with electronic medical records and documentation within 24 hours.
  • Home Health experience.
