Summary of Job Duties: The RN Care Manager manages the care continuum from admission to discharge, ensuring compliance with relevant regulations, collaborating with the multidisciplinary team, and utilizing evidence-based practices.

Qualifications:

Minimum Qualifications:

• Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree
• Three (3) years RN experience with current BLS certification
• Valid Arkansas Registered Nursing License

Preferred Qualifications:

• National Nursing Certification
• BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification

Additional Responsibilities/Competencies:

• Excellent communication skills
• Basic computer skills
• Strong work ethics and accountability

Nursing Process:

• Initiates referrals, establishes diagnoses, coordinates discharge planning, documents communication, and intervenes proactively to ensure timely discharges.
• Assesses patients pre-discharge and refers cases for Ethics Committee consideration as needed.

Education/EBP/Research /QI:

• Participates in quality improvement, evidence-based practice, and research, supporting current evidence integration and positive educational environments.
• Provides patient/family education.

Communication/Collaboration:

• Implements interdisciplinary POC, collaborates effectively, documents care, and reports variances appropriately.

Leadership:

• Acts as a resource, promotes staff development, participates in staffing processes, and develops evidence-based standards.
