Job summary

CareMore Health is seeking a Clinical Case Manager (RN) to manage a caseload of patients with complex medical, behavioral, and social needs. You will apply RN-level clinical judgment to assess, plan, coordinate, and advance value-based care and health equity goals.

You will lead multidisciplinary care teams, coordinate transitions, monitor outcomes, and document activities to support high-quality care and readmission reduction across the continuum.

Qualifications

• RN license and clinical knowledge applicable to complex cases.

Responsibilities

• Conduct comprehensive clinical assessments for patients with complex needs.

• Adjust care plans based on patient condition and escalate when needed.
• Collaborate with payers on utilization management and documentation.
• Maintain accurate case documentation and support quality reporting.
