Humana Inc is seeking an RN Care Manager to join the Family Care Partnership program, serving multiple Wisconsin counties. The role involves performing nursing assessments, coordinating services, and collaborating with an interdisciplinary team to ensure safe, cost-effective care in members' homes and communities.

The position requires WI RN licensure, 2+ years of healthcare experience, travel up to 50%, and availability for a field-based schedule with robust benefits and potential for a bonus

Formación

• Must be a Registered Nurse licensed in Wisconsin.
• Two years of related health care experience.
• Willingness and ability to travel to Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth and Sheboygan counties.

Responsabilidades

• Complete the nursing portion of the comprehensive assessment and input to care planning.
• Monitor wellness and health promotion activities based on assessments.
• Re-evaluate health status at intervals to manage issues and responses to interventions.
• Perform routine health monitoring activities.
• Build constructive relationships with members, families and external staff.
• In crisis, assess and initiate appropriate safety interventions with the IDT.
• Evaluate member ability to manage medications and care with supports.
• Arrange/provide services and monitor effectiveness of interventions.
• Assist with program, policy and QA/QI input.
• Utilize skills to positively impact member care.

Conocimientos

RN license WI

2+ years healthcare experience

Educación

BSN preferred

Case Manager Certification

Medicare/Medicaid knowledge
