Job summary

Humana Inc. is seeking an RN Care Manager to join the Family Care Partnership program in Milwaukee and surrounding counties. You will perform home health visits, complete nursing assessments, and coordinate care to ensure members receive appropriate services.

The role requires an active Wisconsin RN license, at least 2 years of healthcare experience, and willingness to travel up to 50% across Milwaukee and nearby counties. A BSN is preferred and field/home-based work is expected.

Qualifications

• Must be a Registered Nurse licensed in the State of Wisconsin in good standing.
• Two years of related health care experience.

Responsibilities

• Complete the nursing portion of the comprehensive assessment and provide input to the member's care.
• Monitor wellness and health promotion activities based on the assessment.
• Re-evaluate health and wellness at appropriate intervals to manage ongoing issues.
• Perform routine health monitoring activities.
• Build constructive working relationships with members and their families and other staff.
• Provide assessment and interventions in crisis situations to ensure member safety.
• Evaluate ability to manage medications, care interventions, and access to services.
• Arrange for services and monitor effectiveness of interventions.
• Travel to locations within Milwaukee and surrounding counties.
