Job summary

Humana Inc. is seeking an RN Care Manager to join the Family Care Partnership program, serving Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth and Sheboygan counties.

You will perform nursing assessments, coordinate care, and collaborate with an Interdisciplinary Team to ensure quality, cost-effective services for adults in the community. The role requires Wisconsin RN licensure, at least two years of related healthcare experience, and willingness to travel throughout the specified

Qualifications

• Must be a Registered Nurse, licensed in the State of Wisconsin.
• Two (2) years of related health care experience.
• Must be able to travel to any location within the following counties: Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth, Sheboygan.

Responsibilities

• Complete the nursing portion of the comprehensive assessment and input to member care.
• Monitor wellness and health promotion activities based on assessments.
• Re-evaluate health and wellness at appropriate intervals to manage ongoing issues.
• Perform routine health monitoring activities.
• Build relationships with members and family, and appropriate outside staff.
• In crisis situations, assess and implement interventions with other staff for safety.
• Evaluate members' ability to manage medications and treatments with or without supports.
• Provide or arrange services and monitor effectiveness of interventions.
• Assist with program and policy development and input for QI/QA.
