BSN preferred; active RN license required; 1 year of clinical experience required, with 3 years in acute care preferred. ACM/CCM credentials are valued and support care coordination and discharge planning.

Qualifications

• RN license in the state where working or covered by a compact.
• 1 year of experience in a clinical setting is required.
• ACM/ CCM certification preferred.

Responsibilities

• Identify and prioritize patients needing care management using holistic factors.
• Document and hand over plans at care transitions and level changes.
• Support Advance Care Planning, length of stay management and re-admission prevention.
• Coordinate resources and promote timely testing and treatment.
