The ideal candidate holds an RN license with 2–4 years of hospital or related care management experience and excels in collaboration, documentation, and care coordination in a dynamic setting.

Qualifications

• Strong understanding of case management principles, discharge planning, and transitions of care.
• Knowledge of federal, state, and Joint Commission standards related to case management.
• Excellent communication and interpersonal skills to collaborate effectively with patients, families, and interdisciplinary teams.
• Ability to assess complex situations, identify solutions, and implement care plans efficiently.
• Proficiency in EMR and documentation systems.
• Strong organizational and time management skills to prioritize tasks in a dynamic environment.

Responsibilities

• Conducts daily reviews of medical records to assess admission and continued hospital stay.
• Collaborates with IDT to coordinate patient care and resolve care transition issues.
• Develops and implements discharge plans including post-hospital placement and social services.
• Refs cases to physicians or managers when criteria are not met for timely interventions.
• Liaises with community agencies to facilitate patient transitions.
• Facilitates interdisciplinary meetings to support care planning.
• Maintains accurate documentation of case management activities and referrals.
• Assists patients, families, and physicians with discharge planning and post-hospital care options.
• Performs other duties as assigned and maintains reliable attendance.
