AdventHealth in Killeen, Texas seeks an experienced Case Manager RN to coordinate discharge planning and care transitions across inpatient and emergency department settings. The role emphasizes holistic patient care, education for families, and timely service access.

The ideal candidate holds an RN license with an Associate degree (Bachelor's preferred) and 2+ years in medical/hospital nursing, with CCM/ACM certifications preferred.

Qualifications

• RN license required; ADN as minimum degree.
• 2+ medical/hospital nursing experience required.
• Care Management/Utilization Management experience preferred.
• CCM/ACM certifications preferred.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness impacts and discharge resources.
• Assesses readmissions and facilitates discharge planning with the team.
• Documents discharge plans and barriers in medical records.
• Liaisons with payors for post-acute care authorization as needed.
