Qualifications

• Associate's degree in Nursing required.
• Bachelor's degree in Nursing preferred.
• Current RN license.
• 2+ medical/hospital nursing experience required.
• Certification in Case Management (CCM/ACM) preferred.

Responsibilities

• Participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families on emotional, social, and financial impacts of illness.
• Coordinates post-acute care services with providers and payors.
• Assesses readmissions and develops discharge plans with contingency plans.
• Documents discharge planning evaluations and barriers to progression of care.
• Communicates with post-acute care services to ensure continuity of care.
