Qualifications

• Accredited School of Nursing required.
• Minimum of 3 years clinical nursing experience in an acute care hospital.
• Current Washington RN licensure required by date of hire.
• Current BLS (Basic Life Support) certification required at date of hire.

Responsibilities

• Coordinate and facilitate the discharge plan of care and services.
• Review admission data to determine the appropriate level of care.
• Lead interdisciplinary team meetings to improve outcomes and patient satisfaction.
• Ensure accurate documentation supports continued hospitalization and coordination with Payors.
