Job summary

Longview Regional Medical Center is seeking a Registered Nurse (RN) to lead discharge planning and care coordination. The RN will collaborate with interdisciplinary teams to ensure safe transitions of care, review records for medical necessity, and maintain compliance with standards.

The role emphasizes strong communication, EMR documentation, and patient/family support throughout post-hospital care options.

Qualifications

• Associates degree in nursing required
• Bachelor's in nursing preferred
• 2–4 years of clinical nursing experience in hospital/home health/nursing home setting required
• 2–4 years of care management experience preferred

Responsibilities

• Coordinates discharge planning and transitions of care.
• Reviews medical records for appropriateness and medical necessity.
• Collaborates with IDT to ensure communication and care coordination.
• Develops and implements discharge plans and post-hospital placement.
• Refers cases to physicians/managers when criteria are not met.
• Maintains documentation of case management activities and compliance.
• Assists patients, families, and physicians with discharge planning and post-hospital care options.
• Maintains regular attendance and complies with policies.
