Job summary

Trios Health, part of Lifepoint Health, seeks a Case Manager (RN) to lead discharge planning and care coordination for patients in Washington. You will collaborate with the care team, educate patients and families, and ensure appropriate levels of care with payor alignment.

Ideal candidates hold an RN license in WA, have 2+ years of clinical experience, and CCM certification is preferred. This is a full-time day-shift position with competitive benefits and growth opportunities.

Qualifications

• Associate degree required; Bachelor's degree preferred.
• Current state RN license or multi-state licensure for WA.
• CCM certification preferred.
• 2+ years of clinical experience preferred.
• Bilingual and/or English-Spanish speaking preferred.

Responsibilities

• Develop safe, patient-centered discharge plans with the care team.
• Educate patients and families on discharge plans and next steps.
• Coordinate case management strategies aligned with the Case Management Plan.
• Conduct utilization and concurrent reviews with physicians and insurers.
