Providence Medford Medical Center in Medford, Oregon is seeking a full-time RN Care Manager – Social Work to oversee a day shift caseload and coordinate discharge planning. You will assess patients across ages, develop care outcomes, and work with a multidisciplinary team to ensure smooth transitions of care.

The role requires an Oregon RN license and at least 3 years in acute care; BSN is preferred and prior home health, mental health, substance use, hospice, and utilization review experience

Formación

• Oregon RN license required.
• 3 years acute care hospital experience.
• Preferred: BSN; 1 year home health/mental health/substance use/hospice/utilization review; leadership experience.

Responsabilidades

• Develop and manage patient care outcomes for an entire caseload.
• Assess, treat or care for patients across all ages as required.
• Complex discharge planning and utilization review.
