Job summary

Wellstar Cobb Hospital is seeking a registered nurse to serve as a Care Coordinator (PRN/ Night). The role involves coordination of care across the patient’s acute, chronic and long-term stages, discharge planning and utilization management.

The RN must demonstrate advocacy for patients and collaboration with the care team. As a member of Wellstar Health System, you will uphold professional nursing standards, participate in care planning, and support quality outcomes.

Qualifications

• RN license required and practice per Georgia Nurse Practice Act
• BLS certification required upon hire
• Experience in care coordination, discharge planning or utilization management preferred

Responsibilities

• Implements discharge planning and provides resource information
• Coordinates care across acute, chronic and long-term stages
• Assesses patient and family education needs and advocates for care plan
• Documents chart notes timely and accurately in EMR
• Participates in protocol development and performance improvement
• Remains compliant with hospital policies, ethics standards and professional development

Education

Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
