Job summary

Qualifications

• RN license in Georgia and in good standing.
• BLS certification required upon hire (Instructor or equivalent acceptable).

Responsibilities

• Implements discharge planning and provides resource information in a timely manner.
• Identifies and documents barriers for timely disposition.
• Collaborates with health care team to plan appropriate care.
• Educates patient and family about conditions and medical recommendations.
• Supports advance directives and eligibility processes for resources.
• Monitors and follows up on post-discharge needs and continuity of care.
