Job summary

Qualifications

• Must be a licensed RN per Georgia standards.
• All RN Nurses must be licensed and uphold nursing ethics and scope of practice.

Responsibilities

• Implement discharge planning and provide resource information.
• Identify barriers for timely disposition and plan appropriate care.
• Assess and educate patients/families on medical recommendations.
• Perform documentation for care plans and interventions in EMR.
• Participate in performance improvement and protocol development.
• Coordinate care across the continuum for defined patient populations.
• Ensure compliance with policies and payer requirements.
