Qualifications

• Excellent organizational and professional communication skills.
• Ability to review clinical data using screening criteria to determine level of care.

Responsibilities

• Coordinate care across the patient’s continuum during acute, chronic and long‑term illness.
• Discharge planning and resource information to support timely disposition.
• Educate patient and family about plan of care and follow‑up.
• Collaborate with care team to meet quality outcomes and performance improvements.
• Document comprehensive assessments and maintain accurate medical records.
