Job summary

Wellstar Cobb Hospital is seeking a Registered Nurse Care Coordinator for night shift (PRN/PT) to coordinate care across the continuum for a defined patient population. You will perform utilization management, transitional care planning, psychosocial assessment and patient advocacy while aligning with nursing standards and hospital policies.

The role supports a patient-focused approach, collaborating with the care team to ensure quality outcomes and exceptional service for patients and families.

Qualifications

• RN with licensed practice and relevant nursing degree.
• Experience in discharge planning and care coordination.
• Knowledge of utilization management processes and patient advocacy.

Responsibilities

• Implements discharge planning and provides resource information in a timely and efficient manner.
• Identifies barriers for timely disposition and collaborates with care team.
• Provides education to patient/family on conditions and follow-up.
• Assists with advance directives and coordinates post-discharge follow-up.
