Job summary

Qualifications

• Must be licensed as a Registered Nurse in CT.
• Bachelor’s degree in a nursing-related field is preferred.
• Professional certification in case management or clinical specialty is preferred.
• Minimum five years of nursing experience in a medical-surgical acute care setting.

Responsibilities

• Coordinate patient care across the care continuum using independent nursing judgment.
• Identify discharge planning needs and coordinate necessary services.
• Communicate patient’s clinical condition and care needs to the interdisciplinary team.
• Promote nursing professionalism and provide peer support and mentorship.
