Job summary

Cnscares is seeking an experienced RN Case Manager to oversee coordinated care for a patient group. You will manage day-to-day clinical activities in the service area, including in-home visits and collaboration with physicians and payers. Strong documentation and communication skills are essential.

The role requires a current RN license, BLS/CPR, and a minimum of three years of RN experience with at least one year in case management. Home health experience is preferred.

Qualifications

• Associate’s or Bachelor’s degree in a related field.
• Current RN license to practice in each state where patient care is supervised.
• Current Basic Life Support and CPR certification (BLS/CPR).
• At least three (3) years of experience as a Registered Nurse.
• At least one (1) year of case management experience.

Responsibilities

• Provide accurate and complete documentation with rationale for reauthorization of patients.
• Deliver services in the field or away from office up to 50% of the time (in‑home visits, appointments).
• Assist in implementing the Plan of Care and amendments as needed.
• Conduct orientation of new staff and coordinate their start in patients’ homes.
• Serve as a resource for training and follow-up to ensure staff satisfaction.
• Submit nursing documentation timely per policy and complete chart reviews on schedule.
