Zusammenfassung

Cityblock Health seeks an RN Care Manager to support a panel of members with chronic medical and behavioral health needs in the Charlotte community. You will collaborate with care teams to build care plans, conduct assessments, and lead case conferences, ensuring timely monitoring and adjustments to care strategies.

Responsibilities include health screenings, medication reconciliation, and coordination with community resources.

Qualifikationen

• Active, unrestricted RN license in the state of employment
• 3+ years providing clinical services to adults/geriatric with chronic conditions
• Willingness to travel within community for home visits
• Strong documentation and care coordination experience
• Experience in interdisciplinary care teams with community health workers
• Willingness to train in wound care, phlebotomy, and chronic condition care plans

Aufgaben

• Receive members from engagement and care team; communicate program goals
• Complete behavioral health screeners (PHQ-9, GAD-7, AUDIT, DAST-10) during intake
• Conduct in-person clinical exams and place members in programs by intensity
• Lead case conferences and participate in discussions
• Develop care plans with members and address social needs
• Perform regular clinical visits and monitor interventions
• Collaborate with care team for health maintenance and co-occurring disorders
• Perform medication reconciliation, administration, and education
• Address quality gaps and ensure chart documentation (ICD/CPT)
• Use EHRs and scheduling tools to document interactions and track tasks
• Support member care plan goals through coordinated care
